The City of Los Angeles, Office of the Controller, operates a confidential hotline for City employees, contractors, citizens and other interested parties to report fraud, waste and abuse affecting City resources. The City has contracted with an independent company (NAVEX Global) whose trained intake specialists will receive all calls and web-based reports. You may remain anonymous.

Examples:
• Theft of equipment and supplies

These allegations should be referred to the Los Angeles County Department of Social Services Welfare Fraud Investigations Unit (DPSS-Welfare) for investigation.

These allegations should be referred to the Los Angeles County Children’s Group Home Ombudsman.

Violations of the Federal Health Insurance Portability and Accountability Act of 1996.

Examples:
• Inappropriate access to protected health information without a business need.
• Lost or stolen documents, files, medical records that contain protected health information.
• Lost or stolen electronic devices such as laptops, thumb drives, computer equipment, etc., that contain protected health information.
• A patient denied access and/or copies of their medical records without an exception to the HIPAA regulations.
• An individual denied a copy of the covered department’s notice of privacy practices.
• A patient’s request for an amendment to their medical records was denied.
• A patient denied access an accounting of disclosures that the covered department made without his authorization.
• A patient’s special requests regarding confidential communications about their health information was denied.

These matters should be referred to the Los Angeles County Chief HIPAA Privacy Officer.
